He or she will forward the work order to the TV center to complete the request, or contact you with any questions.
This form must be replied to by the faculty member by 1:00 PM the day before the equipment is needed; so make sure you send it in time for them to receive it and reply.
If you are on campus, you may do a paper work order, if it is close to the date of your request.
If your instructor's name does not appear in the list, you must use a paper form.
